ID for all adults in household
Social Security cards for all family members
Birth Certificates for all persons under 18 years of age
Income verification: ALL sources of income (phone #, fax #, and contact person).
8 consecutive pay stubs. SSI/SSDI printout, FIP statement, child support statement (for ALL household members).
Stable income/employment for the past 6 months is required.
Landlord references for the past 5 years: Landlord name, phone number, address of your current and prior residence, how long you lived there. Review of rental history.
NO EVICTIONS IN PAST 24 MONTHS and good rental history.
Assets: Checking and saving account - need bank name, phone/fax, and all account numbers.
Other Assets: 401K accounts, retirement funds, pension funds, IRA accounts, stocks, whole life/universal life insurance, mortgage/deed of trust.
3 most recent consecutive asset statements; asset name, address, phone and fax.
Prepaid Debit Cards: IA FIP card, Reliacard, Direct Express, Citibank, etc.
Current ATM balance receipt or 6 months of statements.
Student status: Over 18 years of age, school contact information (phone/fax), class schedule.
Verify assistance - Pell Grants, scholarships, financial assistance, loans, etc.
Marriage license, divorce decree or notarized statement of separation and plans for divorce.
Pregnancy/Adoption/Legal Custody paperwork
Copy of Section 8 voucher (If you have Section 8).
